Create and publish an inquiry form

Build a public inquiry form, connect submissions to the right pipeline entry stage, test it, and publish its live link.

Available to people with Forms access; changing the owning pipeline or entry stage requires pipeline-structure access.

Create a clear inquiry form that sends every new response into the correct pipeline stage.

Available to

Owners, admins, staff, and front-desk team members with Forms access can create, edit, and publish inquiry forms. Changing a form's owning pipeline or entry stage also requires pipeline-structure access, which ordinary staff do not have.

Before you start

Create the owning pipeline and at least one active open stage first. Every submission enters the pipeline’s Inquiry entry stage. Decide which questions your team truly needs for a useful first response; a shorter form is usually easier for families to finish.

Steps

  1. Open Pipelines, select the pipeline, then open Forms.
  2. Select New form.
  3. Enter the Form name, description, success message, and what should happen After submit.
  4. Add and arrange the questions a family should answer. Select each part of the form to edit its label, choices, and whether it is required.
  5. Review the Owning pipeline and read the displayed Inquiry entry stage. Add an active open stage if the editor says none is available.
  6. Add any default tags or follow-up settings that should be applied consistently to new responses.
  7. Use Test to complete the form as a family would. Fix every validation issue shown by the editor.
  8. Leave Published off while the form is still a draft, then select Save.
  9. When the form is ready, turn on Published and select Save again.
  10. Select Open live to verify the public experience, then copy the Live URL for your website or outreach.
  11. Back on the Forms list, choose one Primary form if this pipeline needs a main entry point. Other forms remain alternate entry points.

What happens next

New public responses create or connect the prospective family information and place the lead in the pipeline’s current Inquiry entry stage. Active automations may react to the submission when their conditions match.

If you do not see this option

Make sure you opened Forms inside a specific pipeline. If New form or Published is unavailable, ask an owner or admin to review your Forms access. If only the owning pipeline or entry-stage control is unavailable, you may have content access without pipeline-structure access.
